You must complete and submit the form if 

• Your plan adjustments impact 20 percent or more of an approved budget, or $5,000, whichever is less
• You are adding a new project or program to your neighborhood engagement plan. The budget is not a factor

You must also submit:

• A revised budget for the funding program you wish to adjust.
• A revised neighborhood engagement plan, as applicable.

You do not have to complete the form if

• Your plan adjustments do not impact an approved budget by at least 20 percent or $5,000
• You are not adding a new project or program to the neighborhood plan

You do have to submit meeting minutes showing board approval of an adjustment.

Past fund

Neighborhood Revitalization Program (NRP) Fund

You must complete and submit the form to request any changes to

• Neighborhood plan funding strategies
• Neighborhood plan language

The process for getting approval to change how you use your remaining NRP funds depends on the amount of money.

Attachments

Submit the following documents as attachment to all plan adjustment requests.

• Board minutes indicating board approval.
• Evidence of community support for any proposed plan modification over $25,000. 
• Evidence of 21-day notification period or a notice to those affected by the change, such as:
  • Social media posts
  • Copies of newsletters
  • Meeting notices
