Rental property owners who want to remove a condemned tree need consent from their renters. Renters can complete and submit this form.
What to know
Your property owner would like to enroll in the Condemned Tree Assistance Program.
To complete the application process, the owner or renters must be either:
- At 80% or less of the Area Median Income established by HUD (2024 80% Income Limit: $97,800)
- Enrolled in an income-verified program, such as:
- EBT
- SNAP
- WIC
- Diversionary Work Plan
- Energy Assistance
- General Assistance
Completing this form is voluntary
The property owner asks that you fill out this form to complete their application.
- Completing this form is voluntary. It will not impact your rental agreement.
