Claim process

You can file a claim if you think the City of Minneapolis or an employee of the City caused harm.

When you can file a claim

File for loss or injury

You can file a claim with the City of Minneapolis if you believe the City or one of its employees is at fault for either:

  • A loss
  • An injury

When you can no longer file a claim

You cannot file a claim if the event happened more than 180 days ago.

How to file a claim online

1. Provide basic details

Include the following information in your online claim form:

  • Your contact information
  • Any information related to property damage or injury
  • The location of the event
  • A description of the event and how the City is involved
  • Contact information for witnesses

Some of the information requested on the online claim form is private and confidential.

2. Include supporting documents

You will need to include all proof that shows fault on the part of the City of Minneapolis.

To file a claim, please gather the following supporting documents:

  • Photos
  • Bills or estimates of repair damages
  • Replacement cost quote
  • Receipts for purchases
  • Other supporting documents

Attach these documents to the online claim you file.

3. Read, sign and submit form

  • Read the official data practices statement included in the form.
  • Sign and date the form.
  • After you submit your completed form, you'll receive a confirmation email.

You can file an online claim if you believe that the City of Minneapolis or a City employee has caused a loss or injury.

What happens after you file an online claim

Investigation

The Risk Management Claims Office receives your claim form.

Acknowledgement letter

  • We'll send you an acknowledgment letter after we receive a completed claim form.
  • We send the letter within 14 days.
  • If you do not receive an acknowledgment letter within two weeks, contact us.

Research and decision 

  • Claims investigators will research the claim. They will gather all necessary information for the Staff Claims Committee.
  • The Staff Claims Committee reviews claims every month.
  • The Committee will base its decision on the investigation. The decision will consider the facts that they have.
  • The City may deny any claim where the City is not at fault.

Appeals

  • If you disagree with the Staff Claims Committee's decision, contact us to schedule an appeal.
  • If you received a denial letter on your appeal, you cannot appeal again with the City of Minneapolis. 
  • Depending on the nature of the claim, you may file a court action by calling Hennepin County.

Data privacy

We may request private data

  • The City of Minneapolis requests some data that may be private under Minnesota law.
  • We request this data to review your claim.
  • You are not required to provide the requested information.
  • Not providing the information could make it difficult for the City to process your claim.

How we use private data

The City needs certain documents to create a claim. 

We will use the collected data to:

  • File a claim
  • Target areas to improve processes
  • Fix unsafe work conditions

Who can see private data you provide

  • Minneapolis Risk Management
  • Other City of Minneapolis staff who process your claim
  • A court upon a court order
  • A state or federal agency, when requested

We can release only public data to the public.
