You must complete either the online or PDF application. You also need to provide the activity report.
What to know
Permit application
You can submit a permit application in two ways:
- Online form, which is our preferred method with secure credit card submission

Activity and analysis report
You must submit an activity and analysis report with your application for permit approval.
- Analyze the blasting surface before starting the work using X-ray fluorescence.
- The X-ray will show if the work will disturb lead-based paint.
